Dear students,
The University of Texas (UT) System requires all international students on non-immigrant visas living in the United States have approved comprehensive health insurance and medical evacuation/repatriation of remains insurance coverage.*
Enrollment in the United HealthCare Student Resources student health plan is mandatory and automatic when we register you for classes. The cost of the student health insurance for the spring/summer 2012 semester is $775 and appears on your tuition and fee bill as Health Insurance. It includes evacuation/repatriation coverage.
Students may be eligible to waive the UT System student health insurance if they have an alternate insurance that meets or exceeds the insurance requirements established by the UT System. You may determine your eligibility by comparing your insurance coverage with the specific insurance requirements and Health Insurance Waiver Application

This insurance waiver must be completed and submitted to the Office of International Programs along with all supporting documentation. Please make your own copies. Applications must be received by 4pm on January 13, 2012. Applications will not be accepted after this date. Faxed or scanned applications will not be accepted.

Also; if you are a scholarship recipient from SACM, WAMS, AND CBIE please make sure to submit a copy of your Financial Guarantee letter (or scholarship letter) to the Office of International Programs along with your Health Insurance Waiver by January 13, 2012. You also need to provide a copy of your FG letter to ESL Services.
IMPORTANT NOTES:*
·**** Your alternate health insurance MUST be in effect from January 17, 2012 through August 23, 2012, which is the entire spring/summer 2012 required coverage period. Students who will not be attending the summer 2012 semester may apply for a refund in March. However, you must submit sufficient evidence that you will not remain in the country during the summer semester.
·**** Scholarship recipients from the SACM may apply for this waiver on a semester basis.
·**** Documents must be in English and must show amounts in U.S. currency.
·**** If your medical coverage meets the requirement but you have not included Medical Evacuation and Repatriation of Remains coverage, the request for waiver will be denied.
·**** All required documents must be attached with your waiver application at the time of submission.
·**** No faxed, scanned, emailed or mailed waiver applications will be accepted. YOU MUST SUBMIT YOUR WAIVER APPLICATION IN PERSON.

If waiver is approved, the student is responsible to verify their ASAP account for the waived insurance fee. If the waiver is not approved, the automatic fee for insurance will remain on students account and must be paid in full along with other education fees.

Dear students,
Welcome to the University of Texas at San Antonio. We are delighted that you have chosen UTSA *as your place to study in United States. Please read this email carefully, as it provides valuable information regarding the Bacterial Meningitis Immunization Requirements.
BACTERIAL MENINGITIS IMMUNIZATION REQUIREMENTS: The State of Texas has required that, in order to register for classes, students under the age of 30 will be required to document their immunization by taking the following steps:
1. Download The Meningococcal Meningitis Vaccine Requirement Form. The Form is on the website Bacterial Meningitis | Student Health Services | UTSA along with specific instructions (please take a look).

2. Gather the required documentation as indicated on the form.
3. Obtain the appropriate health care provider's signature as indicated on the form.
4. Submit the signed form along with the required documentation to UTSA Student Health Services located on the Main Campus at the Recreation and Wellness Center 1.500. Please call (210) 458-4142 with any questions.
*These steps should be completed at least two weeks before you plan to register for classes. Students may take this immunization in their home country. However, the entire immunization record must be in English. It isn’t acceptable to have an English notation to the side about the meningitis vaccine.
************* Students are strongly encouraged to take this vaccination before or by December 31, 2011.
