Please be sure to do the following:
Completely fill out and sign the DS156 form, making sure to type your full name as in the passport, and your correct full address. Please make sure that all data are filled correctly. You will be responsible if it is incomplete or there is an error. The DS 156, & DS157 are available on the Department of State’s travel websiteWelcome to Travel.State.Gov
Your passport must be valid for at least six month from the date of your interview.
If you are a MALE over 16 years old and under 45 years old, you must also complete the DS 157(through the above web site).

(Students with-out a previous appointment can come to the embassy on any interview day (Saturdays, or Mondays, or Wednesdays) at 12:45PM)
You should provide I-20 for you and your dependents.
Print the form or forms. The DS 156 will consist of 3 pages with one being the computer code.
Staple on the form two (5 x 5 cm) color photos on a light background, full-face forward, showing both ears only.

. You should write your full name and your family members' names that are going to travel with you.
The application fee is a non-refundable $131. If your visa is approved, an additional fee will be applied depending on your country of nationality. (Yemeni citizens pay $30.)
