[FONT='Arial','sans-serif']The $50 application fee. [/FONT]
[FONT='Arial','sans-serif']The supplemental social work admission application and statement of purpose. [/FONT]
[FONT='Arial','sans-serif']Two official copies of all undergraduate transcripts in sealed envelopes. [/FONT]
[FONT='Arial','sans-serif']Three letters of recommendation on forms provided by the School of Social Work in signed and sealed envelopes. [/FONT]
[FONT='Arial','sans-serif']Resident Information Sheet, this form should be filled out and submitted regardless of residency status. [/FONT]
[FONT='Arial','sans-serif']Certificate of Immunization. This is required for accepted applicants who have never attended Georgia State University. If admitted, you must submit this form to Georgia State University Health Service (address on form) prior to registration. [/FONT]
[FONT='Arial','sans-serif']For international students who earned their undergraduate degree outside of the United States, an evaluation of foreign educational credentials by an independent evaluation service. [/FONT]

The decision to admit is contingent upon the applicant providing accurate and current information and updating this information as circumstances change. Acceptance into the M.S.W. program depends upon enrollment limitations and size and academic quality of each year’s applicant pool. Meeting minimum requirements is not a guarantee of admission into the program.[/FONT]

You may request registration and test preparation information from the GSU Testing Center by calling (404) 413-1740 or at http://www.gre.org/. In most cases, you can also request GRE information from a four-year college in your area. Allow 4 to 6 weeks for receipt of official scores when taking the computerized test.[/FONT]
